Approved Infant Formula Sources

In accordance with the federal Child Nutrition and WIC Reauthorization Act of 2004, P.L. 108-265, authorized Florida WIC vendors are required to purchase infant formula only from the sources approved by the state.  The sources shown are approved by the Florida WIC Program as of the effective date shown on the document.  If vendors wish to purchase infant formula from a source that is not listed, they must contact the state WIC office in Tallahassee to receive prior approval.  The telephone numbers of the state WIC office are 1-800-342-3556 and (850) 245-4202.   

The list will be updated periodically to show the addition or removal of approved infant formula sources.  It is recommended that vendors check the list periodically to ensure that infant formula is purchased only from sources on the list.
